呼吸道合胞病毒(respiratory syncytial virus,RSV)為肺炎病毒科正肺病毒屬中一種有包膜的單股負(fù)鏈RNA病毒。RSV是引起全球兒童急性呼吸道感染(acute respiratory tract infection,ARTI)的首要病原體,據(jù)估計,>95%的兒童在2歲之前均感染過RSV。RSV感染的臨床癥狀可為普通上呼吸道感染、毛細(xì)支氣管炎、肺炎,重癥者可引起呼吸衰竭、甚至死亡。2015年,全球由RSV引起的急性下呼吸道感染的5歲以下患兒有3 310萬例,其中住院患兒320萬例,死亡5.96萬例。兒童RSV感染給我國醫(yī)療衛(wèi)生事業(yè)帶來了巨大經(jīng)濟負(fù)擔(dān),有研究表明我國RSV相關(guān)住院的平均費用達4025元。
目前利巴韋林仍是臨床上治療RSV感染的主要藥物,但療效并不理想。且利巴韋林氣霧劑可致氣管痙攣,對胎兒有致畸可能。而帕利珠單抗作為唯一已上市的RSV預(yù)防及治療藥物,主要用于高危和免疫缺陷兒童,但由于其成本過高,不適于在中低收入國家推廣使用。目前有19種RSV相關(guān)疫苗及單克隆抗體處于臨床試驗階段,RSV疫苗研發(fā)領(lǐng)域的迅速發(fā)展,說明這一全球健康問題的緊迫性。了解一定時期內(nèi)特定區(qū)域RSV流行情況有助于預(yù)防RSV的感染。
