呼吸道合胞病毒(respiratory syncytial virus)是一種單股負(fù)鏈RNA病毒,是引起嬰幼兒急性下呼吸道感染最常見(jiàn)的病毒病原體,冬春季高發(fā),在全球范圍內(nèi)均有流行。據(jù)估計(jì),2005年呼吸道合胞病毒在全球造成69 000~199 000例兒童患者死亡,99%的病例發(fā)生在發(fā)展中國(guó)家。呼吸道合胞病毒疫苗研發(fā)十分困難,到目前為止,尚未有一種疫苗被批準(zhǔn)上市。為有效控制該病毒的流行,世界衛(wèi)生組織在全球流感監(jiān)測(cè)項(xiàng)目的基礎(chǔ)上,于2017年建立了全球呼吸道合胞病毒的監(jiān)測(cè)網(wǎng)絡(luò),并將其疫苗列入全球優(yōu)先研發(fā)的傳染病疫苗之一。
呼吸道合胞病毒在<2歲兒童中感染風(fēng)險(xiǎn)很高,并且與患兒以后的喘息發(fā)作和哮喘關(guān)系密切;其在老年人和免疫功能低下的高危人群中也有較高發(fā)病率,可延長(zhǎng)患者的住院時(shí)間并導(dǎo)致死亡率增高。研究提示,及時(shí)識(shí)別呼吸道合胞病毒感染的危險(xiǎn)因素對(duì)臨床治療和預(yù)后判斷具有重要價(jià)值。但到目前為止,與此相關(guān)的研究還十分有限,本研究依托北京市呼吸道病原監(jiān)測(cè)系統(tǒng)(Respiratory Pathogen Surveillance System, RPSS),面向不同類(lèi)型醫(yī)院,收集全年齡組范圍的呼吸道合胞病毒肺炎患者,以探索重癥呼吸道合胞病毒肺炎的獨(dú)立危險(xiǎn)因素。
